The following list serves as a general outline of your employee benefits. Benefits vary according to enrollment requirements and full-time, part-time, on call or per diem employment status. Dental Benefits FT 64 & PT 40+ hours	First day of the month after first 30 days	Choose between a DPPO and DHMO option. Both cover preventative, basic, major care, and orthodontia. Offered through CIGNA
Vision Benefits FT 64 & PT 40+ hours	First day of the month after first 30 days	Provides annual eye examination and glasses or contacts every 24 months. Offered through MES.
Basic Life/AD&D Ins Voluntary Life Plans FT 64+ hours Voluntary AD&D Ins FT 64+ & PT 40+ hours	First day of the month after first 30 days	Basic and Optional term life and accidental death and dismemberment coverage available. Coverage for spouse and children may also be elected. Offered through UNUM.
Long Term Disability (LTD) Insurance FT 64+ hours	First day of the month after first 30 days	Basic Long Term Disability (LTD) Insurance coverage available. Optional extended LTD insurance may also be elected
STD/Critical Illness Ins. FT 64+, PT 40+ hours and Per Diem	First day of the month after first 30 days	Income protection in the event of disability and insurance for costs associated with prevention or diagnosis/treatment of cancer and other critical illnesses. Offered through UNUM..
Flexible Spending Accounts FT 64+ & PT 40+ hours	First day of the month after first 30 days	Pre tax savings account for medical and dependent care costs utilizing a debit card. Offered through PayFlex.
Employee Assistance Program (EAP)	Immediately	Confidential counseling and referral services (3 free visits) as well as financial, legal, and concierge services. Offered through PacifiCare Behavioral Health.
Pharmacy	Immediately	Discounts on prescription and over the counter items available in the RCH Pharmacy.
Employee Health Service	Immediately	Select free immunizations for certain employees and treatment and case management for work related injuries.

Matching Funds TSA – 403(b) FT ,PT , and On Call	21 years of age, 1 year of service and 1,000 hours worked	Employee may voluntarily contribute up to 3.5% of plan year wages with 25%-75% employer match based on years of service.
TSA – 403(b)	Immediately	Employee may voluntarily contribute up to IRS maximums.
529 College Bound Savings Program	Immediately	Employee may voluntarily contribute post tax wages, Interest is non taxable when used for college expenses.
Credit Union	Immediately	Arrowhead Federal Credit Union savings by payroll direct deposit.
