The following list serves as a general outline of your employee benefits.  Benefits vary according to enrollment requirements and full-time, part-time, on call or per diem employment status.  Please contact the Personnel Services Department for more information and refer to summary plan description/document and Personnel policies for complete plan/policy information.  Employee benefits may be amended or terminated at any time.

Medical Benefits
FT 64 & PT 40+ hours

First day of the month after first 30 days

Choose between an HMO and a PPO option; both include pharmacy coverage. Offered through Blue Shield.

Dental Benefits
FT 64 & PT 40+ hours

First day of the month after first 30 days

Choose between a DPPO and DHMO option.  Both cover preventative, basic, major care, and orthodontia.  Offered through CIGNA

Vision Benefits

First day of the month after first 30 days

Provides annual eye examination and glasses or contacts every 24 months.  Offered through MES.

Income protection in the event of disability and insurance for costs associated with prevention or diagnosis/treatment of cancer and other critical illnesses. Offered through UNUM..

21 years of age, 1 year of service and 1,000 hours worked

Employee may voluntarily contribute up to 3.5% of plan year wages

with 25%-75% employer match based on years of service.

Continuing Education/Professional Membership Dues
FT 64+ & PT 40+ hours, On Call working 40+hours, & Per Diem

After 90 days of employment

BLS, PALS, CPR, ACLS, NRP, MAB, basic arrhythmia reimbursement.  Classes at RCH are free.  Up to $250 per year available for reimbursement of continuing education fees, licensing fees, and/or professional membership dues.

Pre-Paid Tuition Assistance
FT 64+ & PT 40+ hours, On Call working 40+hours, & Per Diem

After 90 days of employment

$3,500 per calendar year for approved ASN, BSN, and MSN programs – pre-paid.  Applicable expenses include tuition, books, and fees.  A two-year commitment in a full-time, per diem or part-time status of at least 40 hours per pay period with RCH after graduation is required.

Tuition Reimbursement
FT 64+ & PT 40+ hours, On Call working 40+hours, & Per Diem

After 90 days of employment

Reimbursements for tuition up to $2,000 per year

Tuition Payback
FT 64+ & PT 40+ hours, On Call working 40+hours, & Per Diem

After one year of employment beginning 1/1/07

Tuition payback for licensed clinical positions for up to 4 years.  $1,000 per year for 4 years; RNs may receive up to $1,750 for the first 2 years and $1,000 for the next 2 years.

Time off with pay for scheduled vacations, holidays, personal business, or illness.  Annual accrual based on paid hours and years of service.
0-4 years: 24 days
5-9 years:  29 days
10+ years:  34 days

Paid Leave Coordination (PLC)
FT, PT, and Per Diem II

Immediately

Partially compensated time off when disability is covered by California State Disability insurance or Workers’ Compensation (maximum 56 hours accrued)

Bereavement/Jury Duty

FT, PT, and Per Diem II

Immediately

Paid leave in the event of death in the immediate family or if called to serve on a jury.

Blood Donor Club

Immediately

$25 Gift Card for blood donations to the RCH Donor Club.
