Reporting to the Director of Home Health and Hospice, this position collaborates
with the Nurse Educator to promote and ensure employee orientation, competency,
and in-service education programs. This position performs, analyzes, and
reports audit outcomes to departmental leadership, as well as mentoring
to staff on documentation and regulatory standards. Is able to demonstrate
accurate and thorough documentation skills and compliance with documentation
guidelines per departmental standards. This position will demonstrate
proficiency in review and completion of Oasis Assesments and HIS (Hospice
Information Set) according to CMS regulations and industry guidelines.
Provides supervisory visits for Home Health and Hospice field staff per
regulatory guidelines, as well as leadership for Home Health/Hospice electronic
documentation tools and software applications including Horizon and Meditech.
This position will actively participate in Performance Improvement activities
and monitoring of the on-going delivery of care to improve Home Health
and Hospice HCAHPS scores.
--Must have a current California RN License
--Current CPR certification required (American Heart Association only)
--Home Health and Hospice nursing experience preferred
--Excellent understanding of Performance Improvement, Quality Assurance,
and Utilization Management
--Working knowledge of OASIS data collection preferred
