My Health Info provides patients with access to their health information online at any time and from almost everywhere. Available information includes:

• Lab results
• Radiology reports
• Visit history
• Health summary, including allergies, conditions and immunizations
• Discharge instructions
• Instructions for medication
• Upcoming visits

For access to your electronic health records from almost anywhere, gather the following information before signing up:

• Your name, exactly how it appears on your medical records
• Your date of birth
• The email address on file with Medical Records
• Your Medical Record number, found on your discharge packet
